Job summary

Holy Name Medical Center is seeking a Visiting RN to manage home health care for patients, coordinating care plans and supervising Home Health Aides. You will assess patient needs, educate families, and ensure timely documentation. A NJ RN license and 1–3 years of relevant experience are preferred.

Qualifications

• Requires nursing licensure in New Jersey.

Responsibilities

• Coordinate total home care for patients under Home Care/Hospice.
• Perform initial and ongoing nursing assessments.
• Develop and update individualized care plans with goals and discharge planning.
• Communicate care plans and obtain consent for services.
• Supervise Home Health Aides and ensure quality of care.
