Job summary

Upward Health is a home-based multidisciplinary medical group delivering 24/7 whole-person care. The Nurse Practitioner will provide patient-centered primary care primarily virtually, with occasional in-person visits to support transitions of care.

You will lead a multidisciplinary team, perform primary and urgent visits, ensure preventive screenings, and coordinate with PCPs, hospitals, and home care partners to improve patient outcomes.

Qualifications

• Minimum of 5 years of clinical provider experience.
• Multi-state Advanced Nursing License or willingness to obtain.
• Valid driver's license and auto liability insurance.
• Willingness to travel as needed; mostly virtual work.
• Experience in urgent or emergent care is a plus.

Responsibilities

• Triage, diagnose, and treat patients in person or via our virtual care platform.
• Serve as a day-to-day clinical leader guiding the multidisciplinary team.
• Conduct primary care, urgent visits, and annual wellness exams with appropriate screenings.
• Refer patients to behavioral health and specialty services.
• Coordinate care transitions to prevent hospital readmissions.
• Participate in the on-call schedule for 24/7 patient hotline.
