Upward Health, Inc. is seeking a Nurse Practitioner (NP) to deliver high-quality, patient-centered primary care through its Facilitated Virtual Care platform.

The role primarily involves virtual patient care with some in-person visits to support transitions of care and annual wellness visits. ARIA: The NP will lead a multidisciplinary team, manage complex chronic conditions, and participate in a 24/7 patient hotline with after-hours coverage.

Qualifications

• Minimum of 5 years of provider experience, with at least 3 years in primary care or similar.
• Multi-state Advanced Nursing License required or willingness to obtain.
• Valid driver’s license and auto liability insurance.
• Willingness and ability to travel as needed, with virtual-focused duties.

Responsabilités

• Triage, diagnose, and treat patients in person or via virtual care platform.
• Provide clinical leadership and direct the multidisciplinary team.
• Conduct primary care visits, urgent visits, and wellness exams; ensure screenings and quality measures are addressed.
• Refer patients to behavioral health and specialty services as needed.
• Coordinate care transitions to prevent readmissions; collaborate with PCPs, hospitalists, and SNF providers.
• Support 24/7 patient hotline through on-call rotation; after-hours and weekends required.
• Foster a collaborative environment and participate in daily huddles and team meetings.
• Monitor remote patient data from monitoring devices; perform other duties as assigned.
