Upward Health, Inc. is seeking a Nurse Practitioner (NP) to deliver high-quality, patient-centered primary care via our Facilitated Virtual Care platform.

The role combines virtual care with periodic in-person visits to support transitions of care, wellness visits, and risk adjustments for complex chronic conditions. The NP will collaborate with a multidisciplinary team to provide whole-person care to patients in their homes, including urgent visits and post-acute care, with after-hours coverage

Qualifications

• 5+ years’ experience as a provider; min 3 years in primary care or related setting.
• Multi-state Advanced Nursing License or willingness to obtain.
• Driver’s license and auto liability insurance.
• Willingness to travel; mostly virtual work.
• Experience in urgent/emergent care is a plus.

Responsibilities

• Triage, diagnose, and treat patients in person or via virtual care platform.
• Serve as day-to-day clinical leader, directing the multidisciplinary team under Medical Director guidance.
• Conduct primary care, urgent visits, and annual wellness exams with appropriate screenings.
• Refer patients to behavioral health and specialty services.
• Coordinate care transitions to prevent hospital readmissions with PCPs and hospitalists.
• Participate in on-call schedule to support 24/7 patient hotline with after-hours and weekend coverage.
• Foster collaboration within the team and daily huddles.
• Monitor remote patient data from devices.
• Perform other duties as assigned.
