Job summary

Elevance Health seeks a licensed Nurse Practitioner in Ohio to provide primary and urgent care through telehealth and limited in-person sessions. The position requires an MS in Nursing, active Ohio NP licensure (Family or Adult), and 2+ years of experience with complex cases, along with EMR experience and national NP certification.

Qualifications

• Must have an MS in Nursing and an active Ohio NP license.
• Two+ years of experience managing complex care cases.
• Active national NP certification is required.
• Requires a valid, unrestricted Ohio NP license (Family or Adult).
• Hybrid/virtual work with in-person training sessions.

Responsibilities

• Provide primary and urgent care via telephone and tele-video to home and community-based patients.
• Develop and implement care plans for adults with chronic/complex conditions.
• Coordinate with physicians and the care team to ensure timely, quality care.
• Maintain accurate medical records in compliance with state regulations and policies.
• Engage in continuing education as required by state/certifying bodies.
