Elevance Health is seeking an NP Advanced Practice Provider (Acute) with multi-state license to support Medicaid members in a virtual-eligible role. Collaboration with physicians to develop complex care plans, telehealth delivery, and in-person training as required.

Requirements include MS in Nursing, active national NP certification, valid NP license in applicable states, and 2+ years in complex care. Salary range disclosed: $110,080 - $189,888, plus comprehensive benefits.

Qualifications

• Requires MS in Nursing.
• Requires an active national NP certification.
• Requires valid, current, active and unrestricted Family or Adult NP license in applicable states.
• Requires 2+ years of experience in managing complex care cases.
• EMR experience required.

Responsibilities

• Provides primary and urgent health care via telephone and tele video to Medicaid/home-based patients.
• Develops and implements clinical plans of care for adults with chronic/complex conditions.
• Gathers history and exams and develops treatment plans aligned with patient goals.
• Identifies and closes gaps in care.
• Coordinates with interdisciplinary team to meet patient needs.
• Educates patients and families about medications, adherence, and prevention.
• Maintains patient medical records per state regulations and policy.
• Prescribes medications as permitted by state authority.
