Job summary

CareBridge Advance Practice Provider, Nurse Practitioner role available for Ohio and New Jersey licensed NPs with RN Compact. This remote/full-time position offers in-person training sessions and occasional travel from home.

Responsibilities include developing complex care plans, coordinating with healthcare providers, and educating patients. A valid MS in Nursing, NP certification, and 2+ years of EMR-based experience are required.

Qualifications

Requires an MS in Nursing.
Requires an active national NP certification.
Requires valid, current, active and unrestricted Family or Adult NP license in OH and/or NJ.
Requires valid, current RN Compact License.
Requires 2+ years of experience managing complex care cases.
Experience with Electronic Medical Records (EMR).

Responsibilities

Provide urgent health care via telephone and tele-video to home and community-based patients.
Develop and implement care plans for adults with chronic/complex conditions.
Gather history, perform exams, and develop treatment plans aligned with patient goals.
Identify and address gaps in care and coordinate with interdisciplinary teams.
Educate patients and families on medications, adherence, and prevention.
Maintain medical records in compliance with state regulations and policy.
