Qualifications

• Requires an MS in Nursing.
• Requires an active national NP certification.
• Requires valid, current, active and unrestricted Family or Adult NP license in applicable states.
• Requires 2+ years of experience managing complex care.
• Experience with Electronic Medical Records (EMR) is required.

Responsibilities

• Collaborate with physicians and other providers to develop complex care plans.
• Provide primary and urgent care via telephone and telehealth modalities.
• Develop and implement treatment plans aligned with patient goals and conditions.
• Educate patients and families about meds, adherence, and prevention.
• Maintain patient records compliant with state regulations.
