Trinity Health At Home is seeking a Part-time Shift - Home Care & Hospice Virtual Care Center Coordinator Registered Nurse (RN) to support telehealth hospice care. The role involves monitoring patient conditions remotely, coordinating care plans with the interdisciplinary team, and guiding field staff and caregivers in real time.

The position requires an active compact-state RN license, a BSN, and experience with Epic or Home Care EMR.

Qualifications

• 5+ years in Home Care, Hospice, ER or triage as an RN.
• Active RN license in a Compact State (eNLC).
• BSN from an approved nursing program.
• Experience with Epic or Home Care EMR.

Responsibilities

• Conduct virtual assessments and monitor patients remotely.
• Develop and coordinate individualized care plans with the interdisciplinary team.
• Provide virtual education on symptom management and end-of-life care.
• Liaise with field nurses, physicians, social workers and team members for care delivery.
• Respond promptly to urgent virtual consults and triage needs.
• Use telehealth to perform regular check-ins with patients and families.
