Job summary

Amedisys is looking for a licensed RN to support hospice care remotely. The role involves telephonic assessment, care coordination, and patient education, following the established care plan.

Ideal candidates have 1 year of RN experience, CPR certification, and comfort with technology. Willingness to obtain additional RN licenses is reimbursed. Remote, part-time schedule with benefits and state licensure flexibility.

Qualifications

• Current, unencumbered RN license.
• One year of RN experience.
• Current CPR certification.
• Knowledge of terminally ill patients’ needs.
• Comfortable with technology.
• Willing to obtain additional RN licenses in other states (reimbursed).

Responsibilities

• Assess telephonically to determine hospice needs.
• Use EHR and plan of care to develop recommendations.
• Provide education and hospice support via telephone based on plan of care.
• Collaborate with pharmacies, DME vendors and other agencies for patient management.
• Facilitate delivery or maintenance of medical equipment.
• Assign visits and follow-up calls to on-call staff.
• Document in real-time to promote continuity of care.
• Apply standards of practice to achieve pain and symptom management.
