Job summary

Sprinter Health is seeking a part-time Nurse Practitioner to deliver in-home wellness visits and virtual assessments for adults and children. You will provide patient-centered care, perform wellness exams, health risk assessments, and coordinate with the in-home clinical team to ensure seamless service delivery.

Qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in California
• Credentialed Medi-Cal provider in California
• Professionalism, integrity, accountability, confidentiality, and compassionate care
• Willingness to work in a dynamic, problem-solving environment
• Tech-savvy; comfortable with laptops or mobile devices for charting and HIPAA-compliant communication
• Strong written and verbal communication skills
• Ability to work independently or in a team
• Pass national background check and valid clinical license search

Responsibilities

• Start each day by reviewing your caseload and ensuring your technology/tools are ready for patient visits
• Coordinate with Sprinters (in-home clinical team) to visit patients
• Establish rapport and build a warm connection with diverse patients while collecting information and performing services
• Conduct wellness exams, health risk assessments, medication reviews, cognitive screenings, and provide education
• Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs
• Provide comprehensive care coordination and management, including preventive care, referrals, and documentation of findings
