Job summary

Sprinter Health is seeking a Board-Certified Family Nurse Practitioner to deliver in-home and virtual wellness visits. You’ll perform health assessments, blood draws, and care coordination, guided by HIPAA protocols and patient-centered care.

Must hold an active FNP license and be eligible to practice in CA; familiarity with digital charting and remote messaging is a plus. You’ll work autonomously with our in-home clinical team to improve access to care.

Qualifications

• Must hold an active Family Nurse Practitioner license.
• Board Certification as a Family Nurse Practitioner.
• Experience delivering wellness visits and in-home care preferred.

Responsibilities

• Review caseload and prepare tools at start of day.
• Conduct in-home wellness visits and virtual assessments.
• Order or coordinate blood draws and basic diagnostics as needed.
• Perform health risk assessments and medication reviews.
• Coordinate care with in-home Sprinters and document findings.
