Job summary

Medical University Hospital Authority (MUHA) is seeking a Social Worker (MSW) to coordinate discharge planning and assess patients and caregivers throughout hospitalization and follow-up. The role identifies post-discharge risks and applies interventions to reduce adverse events after discharge.

Qualifications

Additional Job Description

Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners.Certification in case management preferred.
