Job summary

Medical University Hospital Authority (MUHA) in Charleston, SC, is seeking a Social Worker (MSW) to assist with discharge coordination and ensure a smooth transition of care. You will assess patients and caregivers during hospitalization and follow-up, identify risk factors, and apply interventions to reduce post-discharge events.

As part of the health care team, you will bridge discharge to outpatient providers, addressing issues and questions with patients and families.

Qualifications

• Master's degree in social work completed.
• MSW licensure by the SC Board of Social Work Examiners is required or preferred.
• Certification in case management is preferred.

Responsibilities

• Coordinate discharge process with the health care team to ensure a smooth transition of care.
• Complete patient and caregiver assessments during hospitalization and follow-up as needed.

Additional Job Description
