Circle Health is seeking a compassionate Virtual Care Coordinator to support CCM patients across the United States. This part-time, remote role focuses on monthly care coordination, documentation, and collaboration with nurses and physicians to keep patients connected and compliant with their care plans.

You’ll work with older adults, coordinate with care teams, and help ensure accurate EMR documentation while maintaining HIPAA standards.

Qualifications

• 1–3 years of healthcare coordination or telehealth experience.
• Bilingual English/Spanish communication skills.

Responsibilities

• Conduct monthly check-ins with CCM patients via phone.
• Review meds, vitals, and goals; update EMR records.
• Track screenings, labs, and follow-ups.
• Coordinate appointments and close care gaps with providers.
• Document activities per CMS CCM guidelines for reporting.
• Identify red flags and escalate appropriately.

About the Role

Circle Health is looking for compassionate and detail-oriented Virtual Care Coordinators to support physician partners and senior-care practices across Florida.As part of the Chronic Care Management (CCM) team, you will help patients with long-term conditions such as diabetes, hypertension, and heart disease remain connected, supported, and compliant with their care plans.This role involves monthly patient outreach, care coordination, documentation, and communication between patients, nurses, and healthcare providers.This is a fully remote role open to candidates residing anywhere in the United States, with training and clinical supervision provided.

Key Responsibilities

• Conduct structured monthly check-ins with CCM patients via phone
• Review medications, vitals, and health goals while updating EMR records accurately
• Track preventive screenings, labs, and follow-up activities
• Coordinate between patients, nurses, and providers for appointments and care gaps
• Maintain accurate documentation as per CMS CCM guidelinesLog activities and time spent for compliance and reporting purposes
• Identify patient concerns or clinical red flags and escape appropriately

Requirements

• Must currently reside in the United States1–3 years of experience in healthcare coordination, telehealth, medical assisting, or related healthcare roles preferred
• Experience in CCM is an added advantageStrong verbal and written communication skills in English and SpanishFamiliarity with EMRs such as Elation, Athena, PCC, or similar platformsCompassionate communication style and comfort working with older adults
• Understanding of HIPAA and patient confidentiality standardsAssociate degree or certification as Medical Assistant, CNA, or LPN preferred
