Chronic Care Staffing is seeking a compassionate and detail-oriented Remote Care Coordinator - RN to support patients managing chronic health conditions through proactive outreach and ongoing care coordination. Responsibilities

Conduct monthly Chronic Care Management (CCM) calls to assess patient health status and provide education.

Requirements:

Must Have

Active, unencumbered RN license.

Active Basic Life Support (BLS) certification.
