Job summary

Advocate Health's Enterprise Pharmacy – Virtual Pharmacy Services is seeking a full-time clinical pharmacist for Acute Care in Charlotte, NC, licensed in NC or GA to start. 32 hours per week with rotating weekends and holidays.

Responsibilities include medication therapy oversight, patient counseling, and collaboration with the care team. You will contribute to formulary adherence, safety and quality initiatives, and supervision of pharmacy staff as assigned.

Qualifications

• Pharmacy degree from ACPE-accredited school required or FPGEC NABP certificate.
• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification may be required.
• Credentialing and privileging required.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Counsels patients and caregivers on medication therapy, adverse effects, and compliance.
• Designs, monitors, and evaluates patient-specific regimens based on literature.
