Qualifications

• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP.
• Licensed in the state(s) of practice or eligible for reciprocity required.
• Two years acute care hospital pharmacy experience preferred.

Responsibilities

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Uses patient education techniques to counsel patients and caregivers on medication therapy, adverse effects, and adherence.
• Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens and documents appropriately.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in management of medical emergencies when applicable.
• Supports medication safety and quality improvement activities and reports events.
