Job summary

Capital Health is seeking a Violence Intervention Program Coordinator to lead hospital-based violence prevention efforts, manage grants, and coordinate community partnerships across the health system. The role oversees policy development, program evaluation, data reporting, and staff coaching.

The position requires on-site work with some evenings and weekends, a background in case management and data analytics, and a commitment to serving diverse urban populations.

Qualifications

  • High School Diploma or equivalent. Equivalent college coursework in social services field preferred.
  • Three years case management experience required. Project management and data analytics experience preferred.
  • AHA BLS. Non-Licensed must obtain before end of orientation period.
  • Demonstrated commitment to working with target population and ability to work with diverse populations.
  • Ability to work on-site full-time and flexible to evenings/weekends.

Responsibilities

  • Uses information technology to access, collect, analyze, maintain and disseminate data.
  • Evaluates data integrity and reliability for analysis and reporting.
  • Defines feasibility and implications of policies, programs and services.
  • Recommends and evaluates policies and programs with program leadership.
  • Provides intensive case management, mentoring and advocacy to the target population.
  • Maintains follow-up contact with clients and service providers; conducts home visits and calls.
  • Documents contacts in records; writes program reports to grant funders.
  • Oversees monthly data entry and report generation.
