Job summary

St. Cloud VA Health Care System is seeking a Primary Care Physician to care for Veterans in St. Cloud - MN. This position is eligible for the Education Debt Reduction Program (EDRP). An incentive is authorized for highly qualified applicants.

Qualifications

• US citizenship or eligible per VA policy; must meet basic requirements.
• MD or DO degree from an approved school; degree earned when studied.
• Current full and unrestricted medical license in the US or DC.
• Residency training approved by VA or GA/AOA; board eligibility.
• Proficiency in spoken and written English.
• Preferred: Primary Care experience.

Responsibilities

• Provide clinical care for Veterans with chronic and acute conditions.
• Participate in huddles and team-based care within the PACT model.
• Manage panels of patients under Primary Care Management Model (PCMM).
• Coordinate care with specialty services and community resources.
• Complete administrative work in accordance with facility policy.
