Job summary

The PCS Social Worker within Social Work Service serves Veterans receiving non-institutional care through the Orlando VA Healthcare System. Duties include reassessing care continuation, coordinating with community providers, and documenting all Veteran contacts in the electronic record.

The role requires collaboration with supervisors and coordinators, attendance at interdisciplinary meetings, and ongoing evaluation of practice with peer review and research activities. Telework is available.

Qualifications

• Education must be recognized by the U.S. Department of Education; foreign credentials require a Certificate of Foreign Equivalency with transcript if used to meet qualifications.

Responsibilities

• Reassess Veterans’ need for continuation of VA-authorized care in the community.
• Serve as liaison between VA and community home health agencies.
• Organize and prepare reports for the Medical Center Director, VISN Liaison, and Central Office.
• Enter Veteran/family contacts in the electronic medical record using standard formats/templates.
• Participate in team huddles, IDT meetings, committees, and matrix staffing discussions; support clinical reminders as appropriate.
