Job summary

You will assess Veterans and their families, connect them with resources, and support treatment planning. This role emphasizes collaboration with medical and mental health staff, delivering interventions across a spectrum of physical and behavioral health needs, and guiding families through complex care journeys.

Qualifications

• MSW required; CSWE-accredited program.
• Eligibility for licensure as a social worker.
• Healthcare or VA experience preferred.

Responsibilities

• Provide care coordination and psychosocial services to Veterans and their families.
• Complete psychosocial assessments to determine needs.
• Collaborate with the PACT team to optimize treatment and independence.
• Offer consultation to treatment staff on psychosocial needs.
• Work schedule: Monday–Friday, 8:00am–4:30pm.
