U.S. Department of Veterans Affairs seeks a Senior Social Worker to provide Home-Based Primary Care (HBPC) and CNH-related social work services. The role includes assessing Veterans and families in home settings, coordinating care, and delivering counseling and case management.

The incumbent will travel within the ICVAHCS area, collaborate with interdisciplinary teams, and ensure care aligns with VA policies and standards. This is a full-time, on-site position with no telework option.

Competenze

Master's degree in social work from a CSWE-accredited program.
Licensure as a social worker at the state level to independently practice social work.
Knowledge and ability to provide psychosocial assessments and counseling in home/community settings.
Strong communication and documentation skills; ability to work with Veterans and families.

Mansioni

Provide social work services in Home-Based Primary Care and Community Nursing Home settings.
Conduct comprehensive psychosocial assessments of Veterans, families, and caregivers.
Deliver individual and family counseling and case management.
Coordinate care and connect Veterans with resources, benefits, and support services.
Act as liaison between VA, care facilities, and community resources; provide guidance to staff.
Travel within the VA ICVAHCS area to home settings as required.
Participate in treatment planning with a multidisciplinary team.
