Job summary

STGi is seeking a Registered Nurse Care Manager to provide clinic-based services at our Community Based Outpatient Clinic in Savannah, TN. The RN will advocate for Veteran patients, coordinate care with the VA Medical Center and stakeholders, and manage patient care under standard clinic protocols.

The role emphasizes patient flow, triage, education, and collaboration with PCPs to ensure quality, uninterrupted care while maintaining safety and confidentiality in a fast-paced outpatient setting.

Qualifications

• Minimum of 3 years of experience in Medical Surgical Unit, Outpatient Facility, Primary Care and/ OR Adult Health.
• Current, unrestricted RN license in the state where practice.
• Current CPR/BLS certification from AHA or MTN.
• Knowledge of nursing principles, procedures, and techniques.
• Ability to assess conditions and respond quickly in emergencies.
• Ability to set measurable patient education goals.
• Ability to conduct individual and group counseling and health instruction.
• Ability to teach home health care activities.
• Knowledge of pharmacology.
• Excellent communication and customer service skills.
• Proficiency with Microsoft Office and EMR systems.
• Must be detail-oriented and able to multi-task.

Responsibilities

• Provide nursing care based on orders to meet patients' physical, mental, and emotional needs.
• Supervise staff in delivering patient care.
• Lead and participate in PACT team huddles.
• Coordinate staffing with PCP and maintain continuity of care between VA facilities.
• Maintain universal precautions and infection control practices.
• Supervise medical assistants and LPNs in data collection and vitals.
• Prepare patients for examinations and assist PACT Providers.
• Ensure smooth patient flow per VA access standards and triage protocols.
• Document patient information in VISTA and EMR systems.
• Assist in scheduling appointments per medical necessity.
