Job summary

STGi is seeking a Registered Nurse Care Manager to serve Veteran patients in a clinic setting, coordinating care with VA medical centers and stakeholders. The RN will provide professional clinical services under the direction of the CBOC and manage patient care.

The role emphasizes patient advocacy, triage, medication administration, and adherence to service excellence standards. Strong communication and team collaboration are essential.

Qualifications

• Current RN license in the state of practice.
• 3+ years in medical-surgical, outpatient facility, primary care or adult health.
• CPR/BLS certification from AHA or MTN.
• Knowledge of nursing principles, procedures, and techniques.
• Ability to assess patient condition and react in emergencies.
• Strong communication and patient education skills.

Responsibilities

• Provide nursing care based on orders to meet patient needs.
• Supervise support staff delivering care where applicable.
• Lead and participate in team huddles and care coordination.
• Ensure adequate staffing with PCPs for quality care.
• Maintain infection control and universal precautions.
• Document patient information in EMR and VA systems.
• Assist in scheduling and triage according to access standards.
• Provide patient education and health resources as needed.
• Assist providers and ensure complete clinical documentation.
• Participate in performance improvement with VA and subcontractors.
