Job summary

Qualifications

• Provide nursing care based on physician, mid-level provider and CBOC Clinic Manager's orders to meet the needs of patients.
• Supervise support staff in delivery of care to patients.
• Lead and participate in PACT team huddles.
• Coordinate care with VA Medical Center and stakeholders.

Responsibilities

• Provide nursing care according to orders to meet physical, mental, and emotional needs.
• Supervise staff delivering patient care.
• Lead and participate in PACT team huddles.
• Coordinate staffing for quality care and continuity between VAMC and clinic.
• Maintain infection control practices.
• Document patient information for quality care and continuity in VISTA.
