Wisconsin Veterans Home at Chippewa Falls is seeking a compassionate Social Worker to join an interdisciplinary team supporting Veterans and spouses in long-term care. The role emphasizes advocacy, care planning, and psychosocial support from admission through discharge, with a Monday–Friday schedule and starting pay of $25.00 per hour.

You will work alongside nursing, therapy, activities, culinary, and other professionals to address psychosocial needs, rights, and quality of life for members

Qualifications

• Active Wisconsin Social Worker license required.
• Excellent communication and interpersonal skills.
• Ability to navigate sensitive situations with professionalism and compassion.

Responsibilities

• Assess and support members' psychosocial needs.
• Develop relationships with members and families as a resource throughout their stay.
• Participate in interdisciplinary care planning.
• Advocate for member preferences, rights, and quality of life.
• Assist with adjustment to long-term care for members and families.
• Provide support related to changes in condition, grief, and life transitions.
• Coordinate services and community resources as appropriate.
• Assist with discharge planning and transitions of care.
• Address concerns and facilitate communication among members and families.
• Maintain timely and accurate documentation.
• Support compliance with long-term care regulations and resident rights.
