Job summary

Paylocity is seeking a Program Coordinator to support daily operations of population health programs. Reporting to the Director of Planning and Population Health, you will coordinate trainings, schedule staff, conduct home visits, and prepare reports and invoices.

You will ensure contractual requirements are met and monitor subcontractors. You will also recruit community partners, deliver trainings and workshops, speak publicly on health topics, and help build partnerships that advance elder

Qualifications

• Associates degree required; BA preferred in health-related field
• 2+ years in Human Services or related field
• Experience working with older adults is highly desirable
• Excellent problem solving, analytical, verbal, organizational and interpersonal skills
• Public Speaking skills is a must; bilingual English-Spanish is required
• Proficient with Word, Excel, PowerPoint, Outlook; internet proficient

Responsibilities

• Coordinate volunteers, trainers, caregivers, subcontractors, and interns
• Provide logistical support to CHWs to ensure program fidelity
• Conduct home visits, assessments and client satisfaction surveys
• Develop and schedule program activities and events
• Develop work plans, track activities and deliverables, and produce programmatic and fiscal reports/invoices
• Monitor expenditures and time cards within budget; document completion of all deliverables
• Interact with internal and external customers to respond to questions about programs and services
• Recruit local community partners, participants, and volunteers
• Deliver trainings, presentations and workshops
• Maintain a strong community presence by speaking to groups about health topics and expand partnerships
• Assist in grants management by identifying opportunities and participating in grant writing
• Develop knowledge of population health issues and elder education strategies
• Assist with disaster response as needed
• Perform other duties as assigned
