Job summary

The Healing Place in Louisville, KY is seeking a Veteran’s Grant Manager to oversee data collection for the Veteran’s Program, manage client consensus, provide case management, and clinical services and support.

Qualifications

• High school diploma or equivalent; bachelor's degree strongly preferred.

Responsibilities

• Track daily census of veterans and maintain current client roster.

• Ensure compliance with VA requirements and inspections.
• Coordinate veteran referrals with VA liaison to programs.
• Collaborate with Coalition for the Homeless on housing programs.
• Provide case management services as needed.
• Conduct psychoeducation classes.
• Coordinate with other departments within The Healing Place.
• Perform outreach to homeless veterans.
• Transport veterans to appointments.
• Participate in VA case conferences and calls.
• Respond to emergency requests with crisis intervention.

POSITION SUMMARY

The Veteran’s Grant Manager is responsible for overseeing data collection the Veteran’s Program, managing client consensus, providing case management, and clinical services and support.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Essential functions include but are not limited to the following:

• Tracks daily census of veterans and maintains daily roster of current clients
• Tracks and analyzes data and outcomes
• Maintains proper documentation in client files
• Maintains complains with VA requirements and inspections
• Works with VA liaison to receive and make veteran referrals to Low Demand and Clinical Treatment Programs
• Partners with VA liaison on veteran service eligibility, housing and goal progress
• Works with Coalition for the Homeless to enroll clients in housing programs and/or monitor progress in housing
• Provides case management services as needed
• Conducts psychoeducation classes
• Coordinates with other departments within The Healing Place as needed
• Performs outreach services to homeless veterans
• Transports veterans to appointments as needed
• Participates in VA case conferences and technical calls
• Responds to emergency requests providing crisis intervention services as appropriate

QUALIFICATIONS/REQUIREMENTS

• High school diploma or equivalent. Bachelor’s degree strongly preferred.
• CADC preferred or ability to obtain CASC in Kentucky required
• Working knowledge of Microsoft Works Suite required (Outlook, Word, Excel)
