Salary Range: $20.00 To $24.00 Hourly

POSITION SUMMARY

The Veteran’s Case Manager assesses the needs of veterans, develops individualized service plans, and coordinates services that support housing stability, recovery, and self-sufficiency. This role works closely with internal teams, the U.S. Department of Veterans Affairs (VA), and community partners to ensure veterans receive appropriate resources, maintain program compliance, and make progress toward their goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Essential functions include, but are not limited to, the following:

  • Assesses veterans to identify service needs, barriers, and personal goals.
  • Develops, implements, and monitors individualized case management plans.
  • Maintains accurate, timely, and compliant documentation in client records in accordance with VA and program requirements.
  • Maintains an up-to-date roster of clients and tracks progress toward housing, employment, and recovery goals.
  • Attends and actively participates in staff meetings, case conferences, and required program meetings.
  • Coordinates with the VA liaison regarding eligibility, housing placement, benefits, and goal progress.
  • Works with the Coalition for the Homeless and other partners to enroll clients in housing programs and monitor housing stability.
  • Conducts scheduled and random room checks to ensure cleanliness, safety, and readiness for move-in or move-out.
  • Connects veterans with community resources, employment opportunities, medical and behavioral health services, and supportive programs.
  • Builds and maintains relationships with veteran service providers and community partners.
  • Serves as a liaison for clients involved in legal, criminal justice, or other external systems.
  • Coordinates services with other THP departments to support holistic client care.
  • Participates in all required VA case conferences, technical calls, audits, and reporting activities.
  • Ensures compliance with VA grant guidelines, documentation standards, and program policies.
  • Facilitates psychoeducational groups, life-skills classes, or recovery-focused programming as assigned.
  • Provides crisis intervention and support as needed.
  • Supports a safe, respectful, and recovery-focused environment consistent with The Healing Place's mission and values.
  • Performs other duties as assigned.
QUALIFICATIONS / REQUIREMENTS
Education and Experience
  • High School Diploma or equivalent required; Associate or Bachelor’s degree in human services, social work, or a related field preferred.
  • Previous case management or human services experience required.
  • Experience working with veterans, individuals in recovery, or people experiencing homelessness preferred.
Skills and Abilities
