Job summary

Primary Care Solutions in the United States is seeking a Nurse Practitioner to serve our Veterans with high-quality care. You will examine patients, document in The VAs VISTA/CPRS system, and collaborate with the Patient Aligned Care Team (PACT) to ensure standards of care.

You will discuss patient outcomes with the Medical Director, complete clinical reminders, and maintain open discretion with confidential information. A rigorous background check is required for employment.

Qualifications

• Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).
• Board-certified in family practice or internal medicine.
• Certification by an applicable professional organization.
• 1 year of clinical medical practice preferred.
• Current, unrestricted license in the state of desired employment.
• Current BLS for providers from the American Heart Association (to be renewed annually).
• Current, unrestricted DEA registration.
• Excellent computer skills.

Responsibilities

• Examine the patient/client and determine the best course of action while documenting in our paperless health program The VAs VISTA/CPRS electronic patient record system.
• Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.
• Discuss patient outcomes, policies, procedures, and records with Medical Director.
• Completes all clinical reminders to keep each patient up to date in the system.
