Job summary

STG International is seeking qualified candidates for the Registered Nurse Care Manager position in connection with an active Request for Proposals (RFP). The RN Care Manager represents Veteran patients in all clinic areas and coordinates care with the VA Medical Center, stakeholders, and the Service Excellence program initiatives.

The RN will provide professional clinical services under the direction of the CBOC, manage patient care, and ensure smooth clinic operations.

Qualifications

• RN Care Manager role requires coordinating care for Veteran patients.
• BSN preferred and NLN-accredited school background desired.
• CPR/BLS certification current.
• Knowledge of patient education and care plans.
• Strong communication and documentation skills.

Responsibilities

• Provide nursing care under physician orders to meet patient needs.
• Coordinate care with VA medical Center and stakeholders.
• Lead and participate in PACT team huddles.
• Ensure adequate staffing for quality care and continuity.
• Administer medications per protocols and monitor patient status.
• Document all patient information in EMR and VISTA systems.
• Assist in scheduling and patient flow per VA access standards.
• Maintain confidentiality and comply with safety regulations.
