Job summary

Catholic Charities Comm's MANA House in Phoenix seeks a dedicated Case Manager to work with transition housing residents. You will meet residents at intake, develop individualized service plans, and assist with housing search, employment readiness, and access to benefits.

You will coordinate with VA services, outreach to community partners, and maintain records in HMIS. Evening/weekend hours may be required; valid AZ driver license and fingerprint clearance needed.

Qualifications

• Education/Background as stated: HS diploma with 4 years relevant experience or Bachelor's in Human Services with 6 months relevant experience.
• Knowledge of Housing First, PTSD, MST, trauma-informed care, crisis intervention, harm reduction, motivational interviewing; sensitivity to diverse populations.
• Ability to work in fast-paced environment; physical demands and travel within area; evenings/weekends may be required.

Responsibilities

• Meet residents within 48 business hours of program entry and conduct intake with ISP and SPDAT.
• Enter all demographic info, case notes, and assessments in HMIS within 48 hours.
• Conduct weekly meetings with residents, review goals and needs; provide guidance on housing and benefits.
• Assist with housing search, employment fairs, benefits appointments, and related activities.
• Liaise with VA and community partners to maximize services for residents.

MANA (Marines, Army, Navy, Air Force) House provides transitional housing for veterans. We provide a foundation for veterans currently or at-risk of experiencing homelessness, providing each one the opportunity to reclaim a life of self-sufficiency.

Case Managers will work with our veterans to build a housing plan to return our residents to stable, permanent housing.

JOB SUMMARY:

Meets with MANA House transitional residents upon initial intake. Provides guidance and assistance to identify goals and actions required for the resident to attain self-sufficiency andstable, permanent housing. Assists the resident with housing search and placement, employment assistance and job readiness, accessing benefits, connecting to physical and mental health providers, connecting to substance abuse counseling, and any other assistance based on individual needs of the resident.

JOB DUTIES:

• Meets resident within 48 business hours of program entry to conduct an intake which includes, creating an Individualized Service Plan (ISP), conducting the initial SPDAT assessment, and assisting resident in developing crisis plan.
• Enters all demographic information, case notes, and program entry/exit assessments in HMIS within 48 business hours.
• Conducts weekly meetings with individual residents and uses motivational interviewing to review goals, progress, and needs.
• Accompanies resident on housing search, employment job fairs, benefits appointments, and any other appointments as needed by the resident.
• Uses progressive or assertive engagement to ensure that each resident is getting the appropriate level of assistance while in transitional housing.
• Conducts street outreach as directed by leadership.
• Interfaces with Veterans Administration Liaison to ensure that each resident is able to take full advantage of services and support options offered at the Phoenix VA Health Care System, and fulfills requirements to participate in the VA per diem program at MANA House.
• ConductMANAHousetoursasrelevant/required,forsupporters,communitypartners andguests.
• Goals focus upon increasing residents’ engagement with MANA , Veterans Administration, supportive services,providers,andprograms.
• Liaise with outside community and veteran organizations to increase resources for clients and to educate the public. Represent MANA atveteranandotherpublicevents.
• Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our veterans.
• Perform other duties as required, including providing back-up support to the Sr. Veterans Case Management Supervisor.

JOB REQUIREMENTS

Education /Background: Minimum of a High School Diploma or equivalent with at least 4 years of relevant experience or a Bachelor’s Degree in Human Services or related field with at least 6 months of relevant experience.

Job Knowledge: 2years’ experienceworkingwithpeople experiencing homelessness, veterans,and/ordisadvantagedpopulations;knowledgein Housing First, Post-Traumatic Stress Disorder (PTSD), moral injury, Military Sexual Trauma (MST), trauma informed care, crisis intervention, harm reduction, motivational interviewing.Knowledgeandsensitivitytodiversepopulations, includinggender,age,ethnicandracialbackgrounds,religious preferences, sexual orientation, and socio-economic status.

Working Conditions/Physical Demands: Must be able to work in a fast paced environment with moderate interruptions. Must be able to stoop, bend, squat, and ambulate over uneven floor surfaces including stairs and steps. Primarily office work, travel within area on a frequent basis, travel to other areas of state as needed. Evening and weekend hours required as needed. Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing. M ay lift up to 25 pounds .

Other requirements

• Valid AZ driver’s license with driving record acceptable to insurer’s standards, reliable transportation and ability to show proof of registration/insurance.
• You must be 21 years of age or older to drive on behalf of Catholic Charities.
• Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.
• Must have or complete within 90 days of hire, training in CPR and First Aid and remain current in the certification.

Hourly rate starting at $22 and may increase depending on experience.

Work schedule: M-F 8am to 4pm
