• Coordinate activities that promote engagement, wellness, and community connection.
• Document client interactions and progress per program requirements.
• Assist with transportation to appointments and resources as needed.
• Collaborate with an interdisciplinary team for coordinated care.
• Maintain professional relationships with community partners and resource providers.
