Qualifications

• English language proficiency as per 38 USC 7403(f).
• Graduate of a school of professional nursing approved by accrediting/accrediting body; ACEN or CCNE; or bridge program pathways as described in VA standards.
• Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state/territory/DC.

Responsibilities

• Provide and supervise direct patient care within the Patient Aligned Care Team (PACT).
• Coordinate care, health promotion, education, and coaching on wellness and chronic care management across the continuum of care.
• Deliver sex-specific Veteran care and maintain care quality and continuity.
