Job summary

U.S. Department of Veterans Affairs in Minneapolis seeks an Acute Medicine Social Worker to join a multi-disciplinary team serving hospitalized Veterans. You will assist with long-term care planning, navigate services, and support referrals across VA and community programs.

The role involves independent psychosocial assessments, crisis intervention, and collaboration with veterans and families to improve health outcomes and adherence to treatment plans within inpatient and outpatient settings.

Qualifications

• Education must be from a U.S. Department of Education recognized institution.

Responsibilities

• Independently perform case finding and outreach for psychosocial needs.
• Provide counseling or psychotherapy for individuals, families, and groups.
• Conduct psychosocial assessments and develop treatment plans with the veteran/family and interdisciplinary team.
• Offer psychosocial interventions and measure outcomes to interventions.
• Coordinate community-based services and referrals to VA and other programs.
• Identify high-risk patients and provide case management services.
• Provide crisis intervention and coordinate family conferences.
• Apply knowledge of medical and mental health diagnoses and treatments.
• Establish therapeutic relationships with veterans and families.
• Provide consultation to staff on psychosocial needs and adherence to treatment.
