Qualificações

• Master’s degree in social work from a CSWE-accredited program.
• Licensure/certification required to practice social work in the state.

Responsabilidades

• Conduct psychosocial assessments and formulate treatment plans.
• Coordinate with internal and external professionals to ensure transitions of care for Veterans.
• Provide crisis intervention and case management for Veterans and families.
• Assist with discharge planning and linkage to community resources.
