Job summary

U.S. Department of Veterans Affairs is seeking a Social Worker for the Tennessee Valley Healthcare System (TVHS).

The role involves managing and coordinating social work services for Veterans and their families across inpatient units, with emphasis on case management, discharge planning, clinical treatment, advocacy, and linkage to VA and community resources. The position requires a CSWE-accredited MSW, state licensure, and relevant clinical experience.

Qualifications

• Master's degree in social work from CSWE-accredited program.
• Licensed or certified to independently practice in state.
• Experience in health care or social work settings preferred.

Responsibilities

• Conduct psychosocial assessments and high-risk screens.
• Participate in discharge planning with interdisciplinary teams.
• Provide individual and family counseling; coordinate community resources.
• Consult with VA staff, residents, and families on psychosocial needs.
• Maintain documentation in CPRS and ensure smooth handoffs.
