Job summary

Centerstone in Florida is seeking a Veteran Care Navigator to provide in-person veteran peer support and care navigation in a multidisciplinary Trauma Recovery Services setting. The role focuses on educating the community about available programming and enhancing access to trauma-focused treatment and support services.

The ideal candidate will be a military veteran with a Bachelor’s degree in a human services field or equivalent experience and must be able to obtain required certifications.

Qualifications

• Bachelor’s Degree in human services or 5 years of behavioral health experience.
• Military veteran status required.
• Willing and able to obtain the certification upon hire.

Responsibilities

• Provides services meeting quality standards and in compliance with regulatory bodies and KPIs.
• Maintains appropriate communication with other employees and works as a positive team member.
• Meets and maintains applicable certification, training, and licensure requirements.
• Represents Centerstone to internal and external stakeholders; develops Trauma Recovery Services programming; provides community education and consultation as needed.
• Works with independence and resourcefulness; seeks additional duties as applicable.
• Supports operations to meet deliverables within the state contract for trauma recovery services; ensures fidelity.
• Participates in process reviews or setup of new program processes to ensure compliance and quality documentation.
• Contributes to management of resource and provider network database.
• Supports integration and coordination across community-based behavioral health care with VA providers.
• Provides targeted case management and referrals for trauma-based counseling with Trauma Recovery Center providers; uses shared-decision making to meet needs.
• Encourages eligible patients and families to utilize Centerstone’s programs; identifies and addresses barriers to engagement using brief, solution-focused emotional support.
• Assesses risk and delivers safety management interventions; makes referrals when needed; ensures safety and crisis resolution.
