Job summary

Harmony At Home Senior Care is seeking a Veteran Service Coordinator to help fellow Veterans access VA-funded home care services. You will educate families, build partnerships with veteran organizations, and guide Veterans through initial VA steps.

Training is provided to ensure success in this mission-driven role. Ideal candidates are Veterans transitioning to civilian work in healthcare or outreach, with strong communication skills and a compassionate approach to sensitive situations.

Qualifications

• Must be a Veteran with honorable service.
• Strong communication skills for families and veterans.
• Comfortable speaking with families during emotionally sensitive situations.
• Organized, reliable, and coachable.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct community outreach focused on Veteran populations.
• Educate Veterans and families on available home care benefits and VA processes.
• Build relationships with veteran service organizations and VA partners.
• Assist Veterans through initial steps of the VA home care process.
• Coordinate referrals with internal care teams and track activity in CRM.
