Hearts & Homes for Veterans (HHV) has served low-income and homeless veterans in Southwest Florida since 2013. In 2025 we served more than 2,700 veterans with over $1.4 million in goods and services. We are growing, and these two roles are part of that growth.
We are hiring two full-time Case Managers to deliver housing-focused case management to veterans and their families through our Supportive Services for Veteran Families (SSVF) program. You will help veterans who are homeless or at risk of losing their housing find stable homes and stay in them.
These positions are grant funded through SSVF. Continued funding depends on annual grant renewal.
Each Case Manager serves about 35 households per year, with an active caseload of roughly 20 to 25 households at any one time.
Veterans are encouraged to apply.
Your time will be split between our Fort Myers office and the field, where you will meet clients in Lee County. A personal vehicle is required, and mileage is reimbursed. Some evening and weekend work is expected.
