Job summary

The Salvation Army North and Central Illinois Division seeks a Health Care Navigator to assist Veterans in gaining access to VA or community health care and to support wellness education. This exempt, full-time role emphasizes case management, enrollment facilitation, and collaboration with health care teams.

You'll conduct screenings, provide education on health topics, address barriers to care, and maintain accurate records in SIMS.

Qualifications

• Possesses a minimum of a bachelor’s degree in related field.
• Advanced degree in social work, healthcare or human services is ideal.

Responsibilities

• Specialized case management and care coordination including SOAR outreach.
• Assists Veterans in enrolling in VA or community health care and coordinating enrollment.
• Supports Veterans in obtaining timely health care appointments and communicating with providers.
• Provides health education and links Veterans to wellness resources and support groups.
• Documents cases in SIMS and maintains accurate records; conducts home visits when appropriate.
• Travel may be required for offsite meetings and engagements; adheres to Safe from Harm policies.
