Job summary

U.S. Department of Veterans Affairs is seeking a Social Worker to join an interdisciplinary team. The role centers on completing psychosocial history and assessments, contributing to care plans and the Minimum Data Set (MDS).

Qualifications

• Master's degree in social work from a CSWE-accredited school.
• Licensure or certification to independently practice social work at the master's level.
• U.S. citizenship required; non-citizens permissible only per VA policy when qualified citizens cannot be recruited.

Responsibilities

• Conduct psychosocial assessments and participate in treatment planning with the Interdisciplinary Treatment Team.
• Provide individual, family, and group counseling.
• Assist with discharge planning and ensure plans are carried out in a timely manner.
• Coordinate services and refer Veterans to community resources as needed.
• Serve as a consultant to clinicians, students, and staff on psychosocial needs and VA policies.
• Maintain effective working relationships with clients, staff, and community agencies.
