The U.S. Department of Veterans Affairs seeks a Social Worker to oversee care coordination, discharge planning, and follow-up for Community Nursing Home Veterans. This role advocates holistic care through education and collaboration with providers to meet patient needs.

Qualifications include a CSWE-accredited MSW, licensure, and strong psychosocial assessment skills. Preference for geriatric/nursing home experience and driver’s license.

Qualifications

• Master's in social work from CSWE-accredited program.
• Licensed to independently practice social work in state.
• Experience coordinating care and discharge planning.

Responsibilities

• Coordinate care management and discharge planning for CNH Veterans.
• Develop and implement psychosocial treatment plans and interventions.
• Coordinate with CNH, PACT, and community programs for continuity of care.
• Document all efforts in the electronic medical record (CPRS).
