Job summary

Aptive Resources is seeking a Community Living Center (CLC) Registered Nurse (RN) to provide skilled nursing care and rehabilitation support to Veterans at the Louis Stokes Cleveland VA Medical Center. The position offers a comprehensive approach to patient care, focusing on rehabilitation and safe transitions back to the community.

The ideal candidate will hold a valid RN license, possess BLS certification, and have experience in skilled nursing or rehabilitation settings. Strong organizational and communication skills are essential for this role.

Qualifications

• Minimum clinical experience in skilled nursing, rehab, long-term care, med-surg, or post-acute care preferred.
• ACLS may be required based on assignment.
• Experience with electronic medical records preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing monitoring.
• Administer medications, IV therapy, and central/peripheral line medications.
• Coordinate care with interdisciplinary teams including therapy.
