Responsibilities

Job description

Responsibilities
  • The primary role of the Case Manager is to coordinate admissions and the after-care needs of each service member, veteran, or dependent based on recommendations of the clinicians, base commands, and patient?s goals. This is a person-centered process which is driven by an individualized treatment plan that functions to maintain the individual?s well-being and prioritize treatment goals and objectives.
  • The Case Manager will familiarize him/herself with community and military resources and draw upon them through linkages and referrals to assist patients in accessing the network of resources that will help maintain their community and/or military status and promote further growth to maximize functional independence.
  • The Case Manager completes all processes, procedures, and case management activities directly related to admission to and discharge from the Military program.
  • The Case Manager will work collaboratively with the Director of Military Programs to ensure timely communication with military bases and referral sources. They will ensure all documents are sent to the appropriate team or agency and all follow-up appointments are annotated on the service member?s discharge instructions.
  • The Case Manager also works collaboratively with clinicians, service members, military and community agencies as needed, with the goal of linking patients to appropriate continuing care resources such as behavioral health, ASAP, Social Work or CAFAC.
  • The Case Manager acts as a member of a multi-disciplinary treatment team, and serves as an advocate for the patient.
Qualifications
  • Bachelor?s level degree in Human Services with at least two years? experience in healthcare setting
  • Must be able to focus on new lead generation, quality referral source development, as well as maintenance of current accounts.
  • Extensive knowledge of military culture
  • Must be able to perform assignments with minimal supervision.
  • Must be able to make sound, independent judgments based on ethical principles.
  • Must have skills in trouble-shooting, flexibility, and prioritizing workload.
  • Must have ability to collaborate with other multidisciplinary team members in an appropriate fashion.
  • Must have effective comprehensive reading skills, strong communication skills, written and verbal.
  • Must have ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
  • Must be able to read, analyze, and interpret professional information, data, marketing trends, and industry-related policy.
  • Must possess a valid state driver?s license with appropriate endorsement.
