Job summary

You will assess patients' environments and support continuity of care on the assigned unit and, as needed, on other units within the facility, ensuring quality, patient-centered care.

Qualifications

• Bachelor's degree in Social Work or equivalent.
• At least 4 years of work experience in the mental health field.
• At least 2 years of work experience in a behavioral healthcare setting, preferably in a psychiatric facility.

Responsibilities

• Coordinates individual, family and psycho-educational groups and drives multidisciplinary treatment planning.
• Performs discharge planning through connection with community resources and environmental assessment of patients.
