Qualifications

• Experience in advanced practice and interdisciplinary team collaboration.

Responsibilities

• Receives and completes consults from interdisciplinary teams and other social workers.
• Develops psychosocial treatment plans with HBPC team and veterans/families.
• Provides individual, family, and group counseling as appropriate.
• Coordinates discharge planning and ensures timely execution of plans.
• Acts as liaison between VA and community resources; educates staff on VA policy and procedures.
• Documents social work interventions in CPRS and maintains patient records.
