Job summary

U.S. Department of Veterans Affairs is hiring a Community Care Coordinator (CCC) RN in Minneapolis to manage care coordination for veterans with clinical triage of moderate and complex needs receiving authorized community care.

The CCC will lead in care coordination, develop individualized care plans, and oversee transitions of care across VA and community providers, ensuring timely follow-up and adherence to guidelines.

Qualifications

• RN with current, full, active, unrestricted nursing license in the US.
• BLS certification required for this position.
• Experience in care coordination, case management, or population health preferred.

Responsibilities

• Review referrals and create care plans for care coordination.
• Coordinate planned and unplanned care with community providers.
• Maintain confidential patient information and ensure safe transitions of care.
• Serve as liaison between VA teams and non-VA facilities.
