Job summary

U.S. Department of Veterans Affairs is seeking a Registered Nurse - Community Nursing Home (CNH) Coordinator to provide skilled nursing care and clinical oversight for Veterans in CNHs.

You will coordinate individualized care plans with Veterans, families, CNH staff, VA providers, and interdisciplinary teams. Responsibilities include leadership in care delivery, discharge planning, nursing case management, and quality improvement while ensuring care aligns with VA policies.

Responsibilities

• Adheres to established policies and leads holistic patient care through collaborative strategies.
• Coordinates individualized care plans with Veterans, families, CNH staff, VA providers, and interdisciplinary teams.
• Provides nursing case management to address health-related self-care deficits of Veterans.
• Performs discharge planning and ensures seamless transitions across inpatient and outpatient care.
